When the laser problem can not be solved by technical support through e-mail or telephone, before sending the laser back to CrystaLaser for repairing, please fill out this form to obtain the RMA number.

Return instructions and policy
1. Fill out this form and send it. You will receive a RMA Number by e-mail from Crystalaser
2. Print two copies of the e-mail including issued RMA number and above information.
Put two copies of the e-mail inside the shipping package together with the laser and power supply.
3. Ship the item with freight prepaid, by air. We will reject the freight collection package.
4. Please use the original package to ship the item to the address:

If you can not find the original package, please well package the item.
Make sure the internal item is safe even the package dropped from 2 meters high to a hard ground.

5. Please only use UPS, FedEx, or Post Office EMS, freight prepaid shipping.
We do not accept any package shipped by Air Cargo, DHL and other carrier from out of the US.
6. Please clearly add the following statement in your commercial invoice for US custom
Please claim the RMA laser need repairing, a value of USD$46 only, in the commercial invoice
"USA made goods return for repairing. Classification No. 9801.00.10.12"
"Original manufacturer: CrystaLaser, Reno, NV, USA"
